This form is to be completed by the Conference Registrar ONLY. Forms received by individual registrants will NOT be processed.
All refund forms will be processed up to the publicized date of Friday, October 10, 2025, regardless of reason.
After the cut off date, refunds will only be processed at the discretion of the
Professional Association Registrar.
